Intake Coordinator (LPN)
Position Overview:
The Intake Coordinator (LPN) is responsible for overseeing the intake process for new and returning patients, ensuring accurate patient data entry, and As part of a mobile wound care team, the Intake Coordinator plays a vital role in facilitating seamless communication between patients, clinicians, and administrative staff. This position requires exceptional organizational skills, attention to detail, and the ability to thrive in a fast-paced, patient-focused environment.
Responsibilities:
Patient Intake & Coordination:
Handle incoming referrals, screen patients, and collect all necessary documentation for new patient registration.
Serve as a liaison between patients, family members, healthcare providers, and internal staff to ensure timely and efficient patient care.
Documentation & Data Entry:
Input and update patient information in the Electronic Medical Record (Athena EMR), ensuring accuracy and compliance with HIPAA standards.
Monitor patient records for completeness and follow up on any missing or incomplete documentation.
Assist in gathering clinical data needed for the billing and coding departments.
Compliance & Quality Assurance:
Ensure all patient intake procedures are conducted according to organizational policies, state regulations, and industry standards.
Participate in continuous quality improvement initiatives aimed at enhancing patient care and operational efficiency.
Customer Service:
Provide excellent customer service to patients and families, answering questions and addressing concerns related to care plans, appointments, and insurance.
Ensure patient satisfaction by delivering timely responses and maintaining professional communication.
Education & Experience:
Active Licensed Practical Nurse (LPN) in the state of employment is required.
Minimum of 2 years of experience in healthcare intake, patient registration, or a related role, preferably within a wound care or home health environment.
Experience with Athena EMR or other healthcare software is preferred.
